What are some examples of PHI?

Any information that can be used to identify an individual is considered PHI.  This includes:

• Consumer's first name
• Consumer's last name
• Consumer's initials
• Consumer's social security number
• Medicaid ID #
• Recipient ID#
• Consumer's medical diagnosis
• Consumer's address
• Consumer's phone number
• Consumer's date of birth
• Consumer's gender
